We'd like to ask you some questions about your alcohol and substance use, as well as other activities. When you are finished, these questions will be compiled in a user friendly, personalized feedback report. In order to produce the most valid feedback, please answer as accurately and truthfully. You will not be asked to provide your name or any personally identifying information on this survey. Information will be kept confidential.
