CAPS Instructions for Online Forms

Below is a copy of the instructions students will see on the homepage of our online forms webpage. Starting July 1, 2008, all students with new appointments must complete the Intake forms online. By 5pm the day before your appointment, students with first-time appointments will receive an email notice from our Appointment Desk about the appointment. The email message will contain a link to our online forms website. Student will then be asked to login to this secure website using their UCSD Single SignOn account and password. From there, you will be given step-by-step instructions. You can download a powerpoint file that includes screen shots with detailed instructions by clicking here! Welcome to Counseling & Psychological Services online forms!

Please read through the Instructions carefully. Completing our online forms involves a 3-step process. Each step must be complete to properly check-in your appointment. This process takes approximately 30 minutes. Do not hit the BACK arrow while completing your forms - your data will not be saved. If you have any questions about our online forms, please call (858) 534-3755 during our normal business hours, Monday-Friday, 8:00am-4:30pm. You can download our instructions by clicking on the following link (or copying and pasting the following URL into your web browser):

IMPORTANT: Your online paperwork session is limited to 40 MINUTES and begins once you have logged in. If you are completing your forms and have exceeded 40 minutes from your login time, your information may not be saved. If you feel you make take more than 40 minutes total, you can logout and re-login after completing Parts 1, 2, and 3 separately.

Please note that your counselor will not be able to see your completed online forms until shortly before your appointment. If you are in crisis and need immediate emergency assistance, call our Central Office during business hours at (858) 534-3755, and (858) 534-HELP outside business hours and holidays.




INSTRUCTIONS:

PART 1:
(a) Click on FORMS on left side menu
(b) Select "Information & Consent Form." Read and electronically sign by checking the checkbox
(c) Select "Acknowledgment of Receipt of Notice of Privacy Practices." Electronically sign by checking the checkbox

PART 2:
(d) When you have submitted both forms, select APPOINTMENTS on left side menu
(e) Confirm your Intake date/time
(f) Select "Complete Questionnaire"
(g) Complete Intake Questionnaire (this takes about 25 min)

PART 3:
(h) When you have completed the Intake Questionnaire, select INTAKE SURVEYS on left side menu to view your surveys
(i) Select and complete the "OQ-45"
(j) Select and complete the "Flourishing Scale"
NOTE: If no surveys are listed, you do not have complete the surveys.

Your online paperwork process is now complete! Please come to your appointment at the date, time, and location listed under APPOINTMENTS. You can now LOGOUT by clicking the option on the top-right of the page.


You must complete these online forms at home or in a private space PRIOR to your appointment. You can use your own computer or any computer that has internet access. Your counselor will not be able to access your information until shortly before your appointment.

Special instructions for non-UCSD student partners: Partners who are not UCSD students but are receiving counseling services (such as partners of UCSD students) cannot complete our forms online. Instead, they can download the Intake Forms Packet (in PDF format) that includes four CAPS forms and an informational document (the NPP). These forms and document are:

  1. The Information and Consent Form which provides information about our counseling service. A second copy is included for you to keep.

  2. Intake Questionnaire which collects information about your past history and current situation.

  3. The Notice of Privacy Practices (NPP). We are required by law (HIPAA) to give you this document and ask you to acknowledge receipt of the NPP. If you want additional information about this law, go to our HIPAA section.

  4. Acknowledgment of Receipt of Notice of Privacy Practices (NPP). Please sign this form to verify that you have received this document. You are not required to read (or print) the NPP prior to your appointment.

In summary, when you meet with your counselor for the first time, please bring the following completed documents:

  1. One completed Intake Form.

  2. At least one signed copy of the Information and Consent Form (you may keep the second copy that is printed for you).

  3. One completed Life Functioning Inventory.

  4. A signed Ackowledgment of Receipt of Notice of Privacy Practices (NPP).
